Departments Admitting International Graduate Students:
Once an international graduate student is been admitted, please complete the Department Funding Form and submit to the International Center (fax: 645-2112, Mail Code: 4240). The form is similar to the previously used page 4 of the Certification of Financial Responsibility.
NOTE: Please complete the Department Funding Form ONLY if the student is admitted.
On this form, the department reports whether or not a funding decision has been made for the student. This information is required before the International Center can issue an immigration form needed for the student’s visa (called an “I-20 for F-1 visa applicants, and a “DS-2019” for J-1 visa applicants). If a funding decision has not been made when the department admits the student, please indicate accordingly on the Department Funding form. You may also include an original copy of the student’s department admission letter or award letter. This will be included in the FedEx shipment of the student’s admission package and immigration document.
NOTE: Due to the recent change in the online admissions application, international graduate applicants will complete information online and submit a Certification of Financial Responsibility form to the International Center directly. Departments will no longer receive or have to forward financial documents from students to the International Center. The Department Funding Form is the only form required from the department.
